Outpatient Physical Therapist - Primary Ortho, Geriatrics

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans based on individual needs, goals, and physician direction. The role requires delegation and direction of all discipline-specific therapy services to ensure the design of functional treatment programs that allow for effective community reintegration. The Physical Therapist will also focus on educating patients, families, students, and coworkers while maintaining high professional standards.

Responsibilities

• Conduct comprehensive patient evaluations and devise appropriate treatment plans aimed at discharging patients within the established length of stay.
• Document patient care in a clear, concise, accurate, and timely manner, ensuring compliance with all state and federal guidelines.
• Facilitate a positive learning environment for students, including supervision and development of learning objectives.
• Create and pursue an individualized professional development plan while actively engaging in continuing education and professional activities.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Participate in performance improvement initiatives to enhance quality of services provided.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for practicing as a Physical Therapist.

Schedule

This position follows a 5x8.0 schedule with 8-hour shifts.

Assignment Duration

This is a long-term contract position, with an expected duration of 13 weeks.
