Overview Physical Therapist Job Summary: We are seeking a dedicated and experienced Physical Therapist to join our team. In this role, you will provide comprehensive physical therapy services to a diverse patient population, including adolescents, adults, and geriatric patients. You will be responsible for evaluating patients, developing individualized treatment plans, and implementing interventions to improve their functional abilities and overall well-being. The ideal candidate will have a strong background in orthopedic rehabilitation and experience managing a caseload of 25-30 active patients. You will be expected to perform approximately 12-13 patient visits per day, utilizing your clinical expertise to deliver high-quality care and achieve positive patient outcomes. The clinic utilizes NextGen Case Management for documentation. Key Responsibilities Evaluate patients\' physical condition, functional abilities, and limitations. Develop and implement individualized treatment plans based on patient assessments and goals. Provide skilled physical therapy interventions, including manual therapy, therapeutic exercise, and modalities. Document patient progress and modify treatment plans as needed. Educate patients and families on injury prevention, rehabilitation techniques, and home exercise programs. Perform Fall Risk Assessments. Collaborate with other healthcare professionals to ensure coordinated and comprehensive patient care. Required Qualifications Minimum of 1 year of experience as a Physical Therapist. Preferred Qualifications Experience with NextGen Case Management. Skills & Competencies Proficiency in evaluating and treating a wide range of orthopedic conditions, including Achilles Tendonitis, ACL Tear, Back Pain (Post-Operative), Calf Strain, Carpal Tunnel Syndrome, Disc Herniation, Fractures/Stress Fractures, Generalized Low Back Pain, Headache, Meniscus Tear, Osteoarthritis, Osteoporosis, Plantar Fasciitis, Postural Dysfunction, Rotator Cuff (Non-surgical Intervention), Rotator Cuff Repair (Post-Operative Care), SI Joint Dysfunction, Stenosis, Thoracic Outlet Syndrome, and Total Joint Replacement. Strong manual therapy skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Proficient in performing Fall Risk Assessments.
