Abdominal Float Transplant Coordinator, Registered Nurse Duke University Health System – Duke University Hospital seeks to hire an experienced Registered Nurse who will embrace our mission of Advancing Health Together. Work Hours & Shifts Full Time: Day shift (8-4:30 pm Monday‑Friday); no weekends, call or holidays required. Department Profile The Abdominal Transplant Programs at Duke University Hospital deliver exemplary care for transplant patients—adult and pediatric—inpatient and outpatient, including kidney, pancreas, liver, intestine and living donor programs. The Abdominal Float Transplant Coordinator works across these programs, providing care through referral, evaluation, acute transplant, and post‑transplant stages and managing care of kidney or liver segment donors. The role involves extensive patient assessment in clinic, phone triage, and patient education to ensure exemplary care. Key Responsibilities Clinical Practice Interpret research and published data for nursing staff and colleagues concerning current issues in transplantation, including clinical, legal, ethical, and economic aspects. Incorporate research related to transplant patient care into evidence‑based practice, collaboratively determining appropriateness with the medical team. Identify areas for study, participate in and/or design research studies. Disseminate findings or information through professional publications and/or presentations. Consultant Serve as a consultant to nurses and other healthcare professionals regarding the care of organ transplant patients. Advise referring physicians/nurses in collaboration with surgical and medical teams. Serve as a community consultation resource through media, telephone, and transplant internet links for information about transplantation and organ donation. Consult with unit leadership to resolve issues relating to transplant patients. Professional Leadership and Development Role‑model advanced practice professional nursing through conduct, appearance, communication, mutual respect, leadership, ethical decision‑making, critical thinking, and problem‑solving skills. Communicate effectively to maintain positive working relationships and achieve desired outcomes. Participate in unit, departmental, and hospital‑wide committees, task forces, or meetings as appropriate. Participate in organ transplant program planning and management within the health system. Demonstrate responsibility for professional practice through active participation in professional organizations, completing a minimum of 25 continuing education hours per year, and actively participating in CSU and departmental meetings and educational opportunities. Enhance the body of knowledge in nursing and solid organ transplantation through exchange of ideas and knowledge using venues such as posters, newsletters, presentations, and publications, including organ procurement organization, patient/families, and the medical team involved (ED, OR, ICU (SICU, ACU), blood bank and histocompatibility labs). Complete necessary documentation according to the United Network of Organ Sharing (UNOS) guidelines including listing, status changes, and post‑transplant follow‑up information. Communicate with multidisciplinary team members and patients and families in a timely manner regarding patient care/issues. Collaborate with social worker and other team members to coordinate discharge plans for patients pre‑transplant or post‑transplant, including referral to home health/IV therapy/hospice/rehab/SNF. Communicate with insurer case manager as needed. Refer patients or families for assistance utilizing health system and community resources as appropriate. Maintain patient transplant record with current data to facilitate communication. Plan nursing care and medical procedures with fiscal responsibility by working collaboratively with transplant administrators, financial coordinators, DUH utilization review, and third‑party case managers. Teacher/Educator Educate patients along the continuum to assist understanding of processes and procedures surrounding pre‑ and post‑transplant issues. Develop patient education materials or identify external resources appropriate for all phases of transplantation care. Educate nurses and other multidisciplinary team members about patient care issues to promote positive outcomes for transplant patients. Provide education and consultation about transplantation and organ donation for the Duke Health System, public organizations, or referral centers/practices. Serve as a resource person/preceptor in organ transplant for graduate and undergraduate nursing programs. Participate in team conferences, rounds, or meetings related to the care of the transplant patient or program issues. Research Utilization and Scholarly Inquiry Provide advanced nursing care for patients before and after solid‑organ transplantation (kidney, pancreas, liver, intestine) along the continuum of outpatient to inpatient Monday‑Friday. Coordinate the evaluation process for pre‑transplant (referral) patients to ensure effective use of hospital and clinical resources. Assess post‑transplant organ function and general health status, collaboratively determining need for additional assessment or intervention. Facilitate the scheduling of studies or procedures for diagnostic purposes, utilizing fiscal responsibility for patient and hospital costs. Participate in the development and implementation of protocols and standards of care for the transplant patient in collaboration with the multidisciplinary team. Assist nurses and other staff to assess, plan, implement and evaluate the care of the transplant patient. Monitor the process and outcomes of routine care to determine quality improvement needs. Key Qualifications BSN required; MSN preferred. 5 years of nursing experience with at least two years in transplant or critical care (or disease‑specific experience). Current or compact RN licensure in the state of North Carolina required. BLS required. Attention to detail and effective communication with physicians, organ procurement agencies, OR, ICU, and ED staff, patients, and families.
