SENIOR LICENSED PRACTICAL NURSE - 50000036 Date: Mar 3, 2026 Requisition No: 871172 Working Title: SENIOR LICENSED PRACTICAL NURSE - 50000036 Location: Lake City, Columbia County Available Shifts: 3:30 PM–12:00 AM (second or third shift eligible for $1.00 / hour shift differential). All shifts eligible for overtime. Paid vacation, sick leave, nine (9) paid holidays, personal day, and tuition waiver up to 6 credits per semester for State Universities and community colleges. ***Other SLPN / GPN positions, including OPS Temporary positions, may be filled from this advertisement within six (6) months of the closing date*** Minimum Qualifications Must possess current licensure as a State of Florida Practical Nurse, or be in current Graduate Nurse (GN) status, possess an eligibility letter from the Florida Board of Nursing and the Authorization to Test (ATT) from Pearson VUE (within 3 months of graduation from an approved program). Must possess current CPR certification. Must be willing and able to work in Lake City, FL. Must be willing and able to work weekends and holidays as required. Successful completion of the due diligence process including, but not limited to, a Level II background check and drug test. Knowledgeable in the use of Microsoft Office software, including Excel, Outlook, and Word. One (1) year of experience working with a geriatric population in a skilled long‑term care facility. Experience with Electronic Medical Records. Duties and Responsibilities Incumbent is responsible for providing quality care to residents to ensure their health, safety, and security in accordance with established agency Standard Operating Procedures and Nurse Practice Act. Perform assigned duties to include, but not limited to, administration of medications and treatments per MD order, monitoring and providing interventions to meet the needs of the resident, completing accurate clinical documentation reflective of the resident’s current status and supervising CNAs and other support staff. Reports to the Nursing Supervisor. Provides administrative support that is technical in nature through established nursing standards, the individualized care plan and interdisciplinary teamwork according to established local and FDVA policies and procedures. Coordinates shift assignments to team members, providing supervision of licensed and unlicensed staff and follow‑up to ensure completion of assignments. Provide assistance with all aspects of resident care, treatments, self‑administration of medications, coordinating medical appointments, and spending time in social interactions with residents each shift worked. Report resident concerns and needs promptly to the appropriate team member. Assist with monitoring and accountability of residents to enable medical personnel to accomplish health–related tasks. Follow Resident’s Rights policies at all times and maintain confidentiality of resident information. Consistently work cooperatively and respectfully with residents, family or resident representatives, facility staff, physicians, ARNPs, consultants and ancillary service providers to assist residents to achieve maximum abilities, safety and promote happiness. Document services and care plan outcomes appropriately within the facility software/database. Participate in staff development and orientation of new personnel. Attend all in‑service programs, training classes and domiciliary meetings. Assume responsibility and accountability for information contained in the Employee Handbook. Observe all facility safety, infection control, risk management, HIPAA policies. Perform other related duties as required.
