The role involves care coordination, discharge planning, and disease management. Applicants must be familiar with admission criteria and the plan of care. Experience with worker's compensation case management is preferred. Candidates must be comfortable working with adult and geriatric patient populations. The position does not require weekend work, but the option to work weekends is available. This is a non-on-call position. The care coordinator will collaborate with interdisciplinary support, including social services. The hospital is a Level II teaching facility with a total of 261 staffed beds. Scrub color is wine-colored, though scrubs are not mandatory. All candidates should confirm experience with Epic on submittal. Previous staff members must include their locations and dates worked in their submissions. Modules are non-billable and approximately four hours, to be completed prior to starting the position.
