Key Responsibilities Analyzes and documents patient responses, evaluates progress, and revises care plans accordingly. Develops and communicates patient and family education plans tailored to individual needs. Implements and modifies effective treatment plans in collaboration with patients, families, physicians, and healthcare team members. Performs assessments including interviews, observations, palpation, and specialized tests using appropriate tools. Independently manages routine to complex patient cases, documents interventions, and maintains accurate clinical records. Organizes patient assignments, participates in case conferences, and collaborates with multidisciplinary teams. Recommends adaptive equipment, home modifications, and discharge planning, including community resource referrals. Performs procedures such as therapeutic exercises, gait training, heat/cold therapy, and ultrasound. Maintains professional growth through participation in educational programs and community outreach. Qualifications And Skills Bachelor's degree in Physical Therapy from an accredited program. 1-2 years of clinical experience. Valid state license to practice physical therapy. Valid driver's license and dependable transportation. CPR certification (BLS and NRP) from AHA or Red Cross.
