Job Title: NP/PA PM&R Clinician – Comprehensive Rehab Consultants (CRC) Position Information Location: Muncie, IN; Parker City, IN; Albany, IN (travel required) Full‑time, 40 hours/week, 8‑hour shift with flexible start times (approx 9AM–4PM) W‑2 role Compensation and Benefits Base salary $112,000 – $132,000 per year Bonus pay and performance incentives Health insurance (Medical, Vision, Dental); short‑term disability, life, critical illness Malpractice insurance covered 100% 401(k) with 2‑3% employer match Paid time off, generous PTO package Continuous education reimbursement $1,000 and CEU credits at no cost Full‑time virtual scribe assistance ($1,368/month value) Provided laptop Responsibilities Deliver specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant at skilled nursing facilities Provide clinical leadership and collaborate with interdisciplinary care teams (physicians, PT, OT, SLP, nurses) Assist PCP teams in managing conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries Track patient progress in therapy, assess barriers, and provide interventions for pain, spasticity, neurogenic bowel/bladder, and other issues Attend multidisciplinary meetings, bridging therapy and internal medicine Engage in ongoing education, monthly didactic lectures, regional team meetings, and annual regional dinners Ideal Candidate Passion for post‑acute care and geriatric population Experience working in SNF or post‑acute setting (preferred) Confidence to work autonomously Demonstrated commitment to continuous improvement and adaptability Strong interpersonal skills and empathy toward patients and facility partners Investment in company culture and team collaboration Qualifications Registered Nurse Practitioner, APRN, or Physician Assistant with active Indiana license Ability to work in the U.S. Experience in post‑acute space preferred Positive attitude and dedication to high‑quality patient care Commitment to 40 hours per week Additional Information Job Type: Full‑time Schedule: 8‑hour shift, flexible within 9AM–4PM; 5‑day work week, days selectable upon onboarding Supplemental Pay: Bonus pay Relocation/Commute: Must reliably commute to one of the listed locations or relocate prior to start License/Certification: NP/APRN/PA Work Location: Muncie, IN; Parker City, IN; Albany, IN #J-18808-Ljbffr
