Home Care Liaison
Addus Family company
Addus Family Company seeks a Home Care Liaison to build and maintain referral relationships, educate community partners on Addus HomeCare services, and support patient transitions to in-home care. In this mission-driven role, you will collaborate with hospitals, clinics, and social service agencies, conduct in-home assessments, and coordinate with operations and clinical teams to ensure safe, timely starts of care. You’ll promote quality, compliance, and patient-centered service while helping elderly, chronically ill, and disabled individuals remain safely in their homes.
