Physical Therapist – Outpatient

Job Description:

PhysioTherapy Associates is seeking qualified Outpatient Physical Therapists to provide comprehensive rehabilitation services. The Physical Therapist will evaluate patients, develop individualized treatment plans, deliver therapeutic interventions, and help patients improve functional abilities and return to community activities.

Candidates must have outpatient clinical experience and be committed to providing high-quality, patient-centered care.

Key Responsibilities:

• Conduct comprehensive patient evaluations and develop individualized treatment plans based on patient needs, goals, and physician direction.
• Implement and modify treatment plans to support functional improvement and timely discharge.
• Provide outpatient physical therapy evaluations and treatments, including 60-minute sessions.
• Maintain accurate and timely clinical documentation, including treatment notes, goals, functional assessments, letters of medical necessity, and billing entries.
• Ensure treatment is provided under valid physician orders and within applicable insurance authorizations.
• Educate patients and families about treatment plans, rehabilitation goals, and safe practices.
• Coordinate discipline-specific therapy services for assigned patients.
• Participate in student supervision and education when assigned.
• Maintain professional licensure and comply with California practice regulations and continuing education requirements.
• Participate in professional development and clinic quality-improvement initiatives.
• Follow patient safety, infection prevention, safe patient handling, and workplace safety procedures.
• Maintain patient confidentiality and adhere to professional ethical standards.

Required Qualifications:

• Degree in Physical Therapy from an accredited school.
• Outpatient clinical experience; the requisition specifies 6 months to 1+ year.
• Active and applicable California Physical Therapist license.
• Current CPR certification.

Preferred Qualifications:

• Previous successful assignment with the submitting agency.
• Previous experience working with Select Medical.
