Responsibilities 60;

• Provide one-on-one 60;Occupational 60;therapy 60;to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care) 60;

• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment 60;

• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence 60;

• Document effectively within FOX’s EMR system 60;in accordance with 60;regulatory standards 60;

• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists 60;

• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place 60;

• Provide telehealth / virtual care when 60;appropriate to 60;extend access and flexibility

Qualifications 60;

• Valid 60;Occupational 60;Therapy 60;license in the state(s) of practice, or eligibility to obtain 60;

• Degree from an accredited 60;Occupational 60;Therapy 60;program 60;

• Basic computer and EMR 60;proficiency 60;

• Current CPR certification 60;

Physical Requirements 60;

FOX clinicians must be able to meet the physical demands of in-home and community-based care, including: 60;

• Exerting medium physical effort, including lifting 25–50 60;lbs 60;occasionally, 10–25 60;lbs 60;frequently, and up to 10 60;lbs 60;constantly 60;

• Performing CPR 60;

• Providing moderate to maximum 60;assistance 60;with patient transfers and bed mobility 60;

• Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities 60;

• Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model 60;

• Wearing PPE when 60;required 60;

In Washington, the standard base pay range for a Full-Time role is $55,000 - $95,000 annually with an opportunity to earn more. Part-Time and FOX Flex (PRN) will be pro-rated.
