Care Manager
Molina Healthcare of Illinois
Molina Healthcare is seeking a field-based, remote role serving members in Hocking County. Provides support for care management and LTSS, coordinating with a multidisciplinary team to ensure high-need members achieve their outcomes across the care continuum.
Travel within the county is expected. Responsibilities include member assessments, waiver enrollment, care plan development, monitoring, and coordination with providers.
