Travel Physical Therapist Opportunity

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Wisconsin Physical Therapist license is required to start the assignment.
• Minimum 12 months of home health experience within the past 3 years is required (client-specific).
• Experience with HCHB (HomeCare HomeBase) EMR preferred.
• BLS certification from AHA or ARC may be required.

Assignment Details

• Facility Type: Home Health.
• Shift Details: Days, 8 hours, Monday through Friday.
• Schedule: Typical hours 8:00 AM to 5:00 PM, Monday through Friday.
• Territory includes Presque Isle, Manitowish Waters, Mercer, St. Germain, Eagle River, Land O' Lakes, and Phelps; clinician must be able to travel within this territory.
• Length of Contract: 13 weeks.
• Call Requirement: No.
