Responsibilities Develop and maintain complex care plans that cater to the unique needs of our clients, working closely with our multidisciplinary team to ensure comprehensive care delivery. Collaborate with clients, families, and healthcare providers to identify and address care needs, concerns, and preferences. Conduct initial and ongoing assessments to evaluate client needs, identify potential issues, and develop targeted interventions. Coordinate and communicate with healthcare providers, family members, and other stakeholders to ensure timely, effective, and efficient care delivery. Identify and address gaps in care, referring clients to community resources and services as needed. Participate in care conferences, contributing to the development and revision of care plans and ensuring continuity of care. Maintain accurate and up-to-date records, including client charts, care plans, and service authorization documents. Demonstrate a commitment to delivering exceptional patient-centered care, adhering to established standards, policies, and procedures. Required Skills and Qualifications Minimum of 5 years of experience in care management, case management, or a related field Strong communication, organizational, and interpersonal skills Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines Familiarity with Medicaid, Medicare, and third-party payers and regulations #J-18808-Ljbffr
