LPN / LVN (Nursing)Location: Local candidates preferred (within 75 miles), primarily in New York.Job Overview:We are seeking a dedicated LPN/LVN to join our team. The successful candidate will be responsible for providing high-quality nursing care to residents in a long-term care (LTC) facility.Unit Information:Unit Type: Long-Term Care (LTC)Number of Beds: 40Common Patient Diagnoses: Multiple Sclerosis, Orthopedic, Dementia, and Memory CareResponsibilities:Administer medications and treatments as prescribed by physicians.Assist in the assessment of residents' physical and emotional health.Document nursing care and patient progress in the electronic medical record (EMR).Provide support and education to residents and their families.Collaborate with healthcare team members to ensure optimal resident care.Participate in the development and implementation of care plans.Assist with daily living activities and personal care of residents.Float within the same building as necessary.Qualifications:Must have a minimum of 1 year of experience in a long-term care facility.Current and active LPN license in New York.Current Basic Life Support (BLS) certification.Skills checklist that aligns with specialty required.Drivers license must reflect the current address.Schedule:Shift: 2:00 PM - 10:00 PM (Block scheduling is not guaranteed.)Guaranteed Hours: 37.5 hours per week.Work Requirement: Every other weekend (EOW) required.Dress Code:Scrub Color: Navy Blue
