Physical Therapist - 13-Week Contract

Day shift, Monday to Friday, 8 AM to 5 PM. Weekend rotations may be required.

Job Description

The Physical Therapist will evaluate assigned patients and develop, implement, and modify treatment plans according to patient needs and physician direction. This role involves delegating and directing therapy services for assigned patients, ensuring the design of functional treatment programs for effective community reintegration, and providing education to patients, families, and peers.

Responsibilities

• Conduct comprehensive evaluations and develop treatment plans focused on improving patients' progress towards discharge.
• Collaborate with interdisciplinary teams to address barriers to discharge, including timely trials of equipment and providing home exercise programs.
• Efficiently manage individual, concurrent, and group treatment sessions based on patient care needs.
• Document patient progress in a clear and compliant manner, including daily notes, goal writing, and discharge documentation.
• Participate in student programs as assigned, fostering a positive learning environment.
• Create and pursue a personalized professional development plan, actively engaging in continuing education and professional activities.
• Engage in patient and professional advocacy activities, including community events and health fairs.
• Adhere to the discipline-specific state practice act and maintain licensure in good standing.
• Demonstrate awareness of organizational performance improvement initiatives and contribute to outcome optimization.

Required Experience / Certifications / Licensure

• Must hold a valid, current state license as a Physical Therapist.
• Minimum of 2 years of experience is required; experience in LTACH settings is highly desired.
• BLS certification is required prior to the first shift.
