Resident Support & Case Coordination Specialist
Jewish Board of Family and Children's Services
The Jewish Board’s Adult Residential Division supports adults with serious and persistent mental illness to live in the community with as much independence as possible. The Case Associate is the intermediate level of direct care staff working with residents in supported housing programs to help residents live in the community as independently as possible. In this role you will carry a caseload, provide person-centered planning, coordinate services with community providers, and document progress #J-18808-Ljbffr Jewish Board of Family and Children's Services
