Registered Nurse Hospice Visit PRN

Transitions Care

Transitions Care is seeking a PRN Hospice Visit Registered Nurse to provide compassionate in-home care for patients with serious or terminal illness. In this role, you’ll conduct skilled nursing visits, perform comprehensive assessments, manage symptoms, administer medications, and coordinate care with an interdisciplinary hospice team. You’ll educate and support patients and families on disease progression, comfort-focused care, and end-of-life planning while documenting accurately and complying with hospice, regulatory, and safety standards in a mission-driven, patient-centered environment.
