Coordinate with multidisciplinary teams to ensure smooth discharge workflows. Provide case management services aligned with unit policies and patient needs. Document assessment and care plans accurately within EMR systems such as Meditech and CarePort. Communicate effectively with patients, families, and healthcare teams to facilitate care transitions.
