Job Summary

We are seeking a skilled Physical Therapist to provide rehabilitation services to patients in a rehab facility. The PT will evaluate patients, develop individualized treatment plans, and provide therapy to improve mobility, strength, balance, and functional independence.

Responsibilities

• Perform physical therapy evaluations and assess patients’ functional abilities.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercises, gait training, balance training, and mobility activities.
• Monitor patient progress and adjust treatment plans as needed.
• Document evaluations, treatments, progress, and discharge recommendations.
• Educate patients and families on exercises, mobility, and safety.
• Collaborate with nursing staff, physicians, OT, SLP, and other healthcare professionals.
• Participate in care planning and discharge planning.
• Maintain compliance with facility policies, safety standards, and infection-control procedures.

Qualifications

• Active Pennsylvania Physical Therapist (PT) license in good standing.
• Graduate of an accredited Physical Therapy program.
• Previous rehab, SNF, LTC, or post-acute care experience preferred.
• Current BLS/CPR certification preferred.
• Strong clinical, communication, documentation, and organizational skills.
