Office Based Nurse Practitioner- Pediatric Hematology/Oncology

Hematology/Oncology APRN Position

Seeking an experienced Hematology/Oncology APRN to support our bleeding disorders, benign hematology, and survivorship/late effects programs. Responsible for providing comprehensive care at an advanced practice level to pediatric patients with bleeding disorders and benign hematologic disorders through assessment, diagnosis, clinical management & treatment, collaboration, coordination, education, and research. Serves as the KIDZ Clinical Hemophilia Treatment Center and 340B Clinical Coordinator. Also provides Nurse Practitioner support for Survivorship clinics in coordination with multiple specialties.

Duties and Responsibilities

Outpatient Care

• Under indirect supervision and working interdependently with physicians, nurses, and other healthcare providers, provides a broad range of nurse practitioner services to patients referred to the Comprehensive Hemophilia Treatment Center for evaluation of a possible inherited bleeding disorders or other benign hematological disorders.
• Performs a variety of functions and technical procedures necessary for assessment, diagnosis, treatment, follow-up, and outcome evaluation.
• Performs physical examinations and specialized procedures under the direction of a physician
• Obtains/interprets laboratory and diagnostic tests and communicate findings to the families and medical team.
• Charts all patient transactions in accordance with policies and procedures; writes orders and records progress notes that will be reviewed and countersigned by supervising physician.
• Formulates a family-centered health care plan in collaboration with other team members and documents the plan in EMR in a concise, physician targeted manner that facilitates patient adherence and outcome evaluation.
• Evaluate responses to interventions and communicate these results to the family and team and modify plan of care as needed.
• Utilizes written guidelines/protocol for clinical decision making and management.
• Writes orders for medications, laboratory work, and diagnostic tests.
• Responds to patient calls/questions forwarded by clinical team promptly and provides advice on clinical issues.
• Generates letters of medical necessity, and other documentation as needed for patients.
• Provides clinical assistance to team in obtaining prior approval from insurance companies for procedures when clinical information is required to complete the process.
• Adjusts to fluctuating peaks in patient flow, acuity, and other operational demands while maintaining quality.

HTC Administration

• Assumes, along with other providers, a role of liaison for health care decisions between the HTC physicians and the health care providers in the community, including Hemophilia of Georgia Inc, local pediatricians, internists, blood bank directors, dentists and other providers.
• Serves as an educational resource for patients, families, health care professionals, students, etc. This may involve informal teaching, development of written educational materials, didactic lectures, attendance at educational meetings, etc.
• Participates in administrative meetings by providing suggestions and constructive feedback on improvements to care and the efficiency with which the program operates.
• Participates in the design and implementation of program policies, operating procedures, clinical guidelines and other issues as they arise.
• Represents the H/T Program in outside clinical and administrative initiatives, e.g., improving the process/flow for ordering clinical labs, improvements to the electronic medical record, etc.
• Supports programmatic initiatives that are directed at quality improvement and programmatic development.

• Maintains other appropriate continuing education as needed to comply with federal regulations.
• Performs other related duties as assigned by management.
