Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Ohio PT license
• Home Care experience
• PT certification

Assignment Details

• Shift hours: 8:00 PM - 4:00 AM
• Shift days: Monday through Friday
• Schedule: 8 hours per day, 5 days per week
• Coverage area includes Lucas County and surrounding areas
• Mileage is reimbursed between visits, from first patient to last patient
• Productivity expectation: 30 points
• Primary responsibility: Case Manager
• Dress code: Blue/Black

Holland, Ohio offers convenient access to both suburban amenities and outdoor recreation. The community sits about 10 miles southwest of downtown Toledo and provides quick access to the Toledo Museum of Art and the Toledo Zoo.
