Nurse Practitioner- Primary Care- Upper East Side Manhattan AdvantageCare Physicians (ACPNY) is a primary and specialty care practice serving half a million patients across the New York metropolitan area. Summary of Position Nurse Practitioners provide medical services in collaboration with a physician, for patients in primary care as well as in medical and surgical specialty areas. Principal Accountabilities Obtains and documents patient history as appropriate to chief complaint or reason for visit. Conducts relevant physical exam and evaluates patient, documenting pertinent positive and negative findings. Diagnoses and treats disease and manages chronic illness. Prescribes medication in accordance with New York State and federal law. Orders appropriate laboratory, radiological and special examinations or tests; interprets results Provides preventative health services, including health education and counseling. Collaborates with other clinicians and staff members to ensure continuity of care of patients. Performs other job-related duties as required. Qualifications Education, Training, Licenses, Certifications Bachelor’s Degree (R) Graduate of a Nurse Practitioner education program registered by the NYS Education Department (R) Master’s Degree (R) Licensed and currently registered as a Registered Nurse by NYS Current New York State Registration as Nurse Practitioner Current Board Certification as an NP from a NYS approved accrediting body Relevant Work Experience, Knowledge, Skills, and Abilities 1-3 years of related experience (P) Experience working with diverse populations in an ambulatory healthcare setting.
