Congestive Heart Failure Nurse (Per Diem)

Position Overview When you join the growing BILH team, you’re not just taking a job—you’re making a difference in people’s lives. Per diem requirement is 2 days per month but coverage typically needed 4–5 days per month, with most shifts being 8 hours. Job Responsibilities Provides front‑line support to patients and staff in the Advanced Heart Failure Program, coordinating disease‑management activities in both inpatient and ambulatory settings for a subset of high‑risk heart‑failure patients. Collaborates with physicians, nurse practitioners, inpatient nursing staff, hospital staff, and the patient to coordinate all aspects of post‑hospitalization care, including in‑hospital patient teaching and evaluation prior to discharge, a 72‑hour post‑discharge call, medication titration and reconciliation, and scheduling of follow‑up appointments with HF staff post‑discharge. Triages patient and provider calls, including interval phone assessments, coordination and follow‑up of laboratory tests, refilling cardiac medications, and consultation with heart‑failure staff when needed. Acts as a resource to the administrative and clinical staff to assist in the management of patient care and scheduling issues both in inpatient and ambulatory settings. Provides education to cardiology nurses and inpatient nurses on heart‑failure management, pharmacology, and common patient issues. Uses good clinical judgment and decision‑making skills and assists inpatient nurses with clinical assessment, management, and education of heart‑failure patients. Updates clinical knowledge and skills through consultation, formal and informal education, and review of literature. Identifies process‑improvement opportunities related to the heart‑failure patient population and participates in improvement initiatives. Works effectively with all disciplines to ensure the best outcomes for patients. Pay Range $36.00 – $90.14 per hour. This is the base hourly wage range; actual compensation may be higher based on shift differentials, call pay, premium pay, overtime pay, and other applicable factors. Requirements Per diem requirement of 2 days per month, typically needing 4–5 days per month. All staff must be vaccinated against influenza as a condition of employment.
