PACE Personal Care Attendant

Primary Accountability The Personal Care Attendant is under the direction and supervision of the Center Manager. The Personal Care Attendant provides personal care services to FHCN PACE participants in the day health center, as well as other activities and tasks as assigned to assist the primary care, therapy, and adult day health staff. Description of Primary Responsibilities 1. Provides personal care and escort assistance in accordance with individual treatment plans – including peri-care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation. 2. Reports observed changes in participants’ conditions to the Center Manager and/or other center-based clinical staff. 3. Proactively communicates participant health and safety concerns to the Center Manager and/or other program staff. 4. Demonstrates safe practices utilizing proper body mechanics and complying with universal precautions. 5. Interact in a friendly, courteous, and professional manner with participants and their families/caregivers. 6. Participate in social and recreational activities with participants as appropriate. 7. Implements interventions, as appropriate to training, as directed by rehabilitation staff, including the use of mechanical assistance for participant transfers and mobility. 8. Occasionally accompanies participants to outside clinical appointments, when necessary. 9. Assist with food service delivery and monitoring during mealtimes. 10. Responsible for adhering to the Attendance and Absenteeism Policy, recognizing that regular attendance is considered an essential function of all FHCN positions. Absenteeism is not being at work or failing to attend a paid workshop, training, or event unless the absence is protected by law. 11. Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need. 12. Other projects and duties as assigned. Description of Primary Attributes Professional & Technical Knowledge: Effective June 1, 2026, all individuals hired into the role must: 1. Possesses work-related skills at a higher level than completion of high school, including formal written and verbal communications skills, computational and computer skills, mathematical, and technical skills. 1. Must have proof of completion of high school with a minimum overall GPA of 2.5 or completion of General Educational Development (GED) with a minimum overall score of 162.5. 2. If an individual has completed a degree at a higher level than required by the role and had a stronger GPA in that program, they may provide proof of GPA from that degree in lieu of the high school diploma. 2. An HHA or CNA license is strongly preferred. 3. Must have 2 years of clinical experience as a Medical Assistant, Personal Care Aide, Certified Nursing Assistant, or EMT. 4. 1 year experience with the frail or elderly population. 5. Must complete and pass the FHCN PACE approved curriculum for Personal Care Aides upon hire. 6. Communicate effectively with a variety of people, including participants, families, staff, and other agencies. 7. Work effectively in a team environment and independently with participants. 8. Care for geriatric patients in a compassionate and patient manner. 9. Only act within the scope of his/her authority to practice. 10. Meet a standardized set of competencies for the specific position description established by FHCN PACE and approved regulatory agencies before working independently. 11. Conduct daily physical activities to assist frail elders in an ergonomic manner to avoid injury. 12. Receive and carry out orders from the Interdisciplinary Team regarding personal care services and/or monitoring of participant status. 13. Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact. 14. Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse. 15. Complete the competency profile prior to providing direct participant care. Technical Skills: 1. Ability to prepare more complex documents in Microsoft Word, including creating tables, charts, graphs, and other elements. 2. Ability to use Microsoft Excel to review and compile data, including the use of formulas, functions, lookup tables, and other standard spreadsheet elements. 3. Ability to create basic presentations in Microsoft PowerPoint. Licenses & Certifications: Currently certified in cardiopulmonary resuscitation (CPR), if CPR is not current, will be granted 30 days from the date of hire to schedule for the next available CPR certification class provided by FHCN. Communications Skills: 1. Job duties require the compilation of information prepared in effective written form, including correspondence, reports, articles or other documentation. 2. Effectively conveys technical information to non-technical audiences. Physical Demands: Physical demands: While performing duties of the job, the employee is required to move about the FHCN PACE center and actively engage with the participants. Employees must be able to sit for extended periods of time, as well as work at the computer for long periods. Employees are required to use their hands and fingers, especially for typing on the computer and using the mouse. Employees must be able to talk and listen, particularly for phone communications. Work Environment: Employees must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the FHCN PACE members’ care. The work setting is the Adult Day Health Center and includes a primary care clinic environment with moderate noise levels and controlled office temperatures. Pay Scale: Min: $21.25 Max: $29.33
