Key Responsibilities

• Perform home health visits including Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and routine visits
• Treat an average of 6.5 patients per day across assigned home health territory
• Work with patients presenting with diagnoses such as CHF, orthopedic conditions, and pneumonia
• Operate on a 5-day, 8-hour day schedule with rotating weekend coverage (8:00 AM – 5:00 PM)
• Participate in holiday rotation as required
• Utilize a company-provided phone and tablet to support field-based documentation and communication

Required Qualifications

• Active Physical Therapy Assistant (PTA) licensure
• Current Basic Life Support (BLS) certification
• Ability to independently conduct SOC, ROC, recertification, evaluation, and daily visits
• Willingness to participate in rotating weekend and holiday schedules
• Comfortable working in a home health / field-based setting

Preferred Experience / Skills

• Prior experience as a traveling PTA preferred; first-time travelers will be considered
• Experience treating patients with CHF, orthopedic, or pulmonary diagnoses in a home health setting
• Candidates beyond new graduate status preferred
