Description

• Assess patients in both inpatient and outpatient settings, crafting tailored treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and functional capabilities
• Work closely with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide patients and their families through therapeutic objectives, discharge preparations, and home exercise regimens
• Maintain accurate documentation of evaluations, treatments, and progress, adhering to facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure synchronized patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications in areas such as neurology or geriatrics are beneficial but not mandatory

Skills

• Prior experience is highly preferred
