Hospital Liaison Lead (62281)

Job Details Location: Stith Center, 791 Chambers Road, Aurora, CO 80011. Position Type: 1 FTE (40 hrs/week, benefited). Education Level: Bachelor’s Degree. Salary Range: $22.04 - $33.07 per hour. Shift: 3 Day Hybrid – 3 days Remote. Bilingual Language Differential Pay Employees who successfully pass a language assessment receive a fixed amount of $3,000 per calendar year, paid bi‑weekly across 26 pay periods, prorated based on hire date and FTE. Eligibility and payment are subject to language requirements of the position and demonstrated proficiency. Salary Starting wage: $22.04 - $33.07 per hour, paid bi-weekly. Essential Functions Coordinate with manager and director to develop, implement, and maintain a training process for role‑specific skills of all hospital liaisons. Support onboarding and training for all other hospital liaisons. Develop workflow for organizing and assigning tasks and shared responsibilities among the hospital liaison staff. Organize and delegate hospital assignments for care coordination. Assume primary role for ensuring completion of hospital liaison data reports, including daily data reports to Colorado Access (business days), delegating when appropriate, and ensure completion of the Fort Logan fiscal year summary report. Manage the e‑fax and email referral system ensuring that referral reports are properly received, distributed among the liaison staff, and responded to daily. Serve as primary point of contact representing the hospital liaison staff to external collaborating agencies. Provide administrative support and oversight to the hospital liaison staff regarding collaborative process steps for working with external hospital systems. Receive communication from referring treatment agencies via phone, email, or fax. Respond to referring individuals and agencies in a timely fashion, usually within the same day, to discuss referral needs. Provide a level of care assessment and determination for purposes of uniting clients with the treatment team that will provide the level of care needed upon discharge from the external treating facility. Monitor the e‑fax for clinical referral data, upload the data to the client’s electronic record, and forward the clinical referral data to the assigned treatment team. Maintain a data tracking record with the referral information to include the day of referral and scheduled follow‑up appointment. Work with assigned hospitals to coordinate referrals of clients from acute psychiatric treatment units to other facilities for more intensive and longer‑term services. Interact with the Colorado Access liaisons to coordinate treatment and meet the 7‑day post‑hospital follow‑up engagement goal. Maintain regular communication with Aurora MHC treatment teams to be fully aware of the internal referral processes used to set clients up for needed services. Occasionally provide education to internal and external referring individuals to assist in a smooth and collaborative referral process. Work closely with primary supervisor to problem‑solve referral issues as they occur, occurring in real time to expedite successful referral to post‑hospital and ATU services. Coordinate the referral process for certified clients, including assisting with setting up the paperwork referral components, scheduling doctor‑to‑doctor consultations for referral of certified clients, and working with the intensive services teams to coordinate the team assignment. Qualifications and Requirements Bachelor’s degree required. Master’s degree in a clinical human service preferred. No previous experience required; strong interest in working in mental health arena. Knowledge of theories and evidence‑based practices related to clinical treatment. Solid understanding of mental illness and treatment modalities within a multidisciplinary approach. Required Vaccination and TB Test All employees are required to receive an annual influenza (flu) shot and present a negative TB test result before their first day of work. Proof of both the flu shot and negative TB test must be provided on the first day of employment. Medical and religious exemptions can be requested if necessary. Benefits Health insurance: Kaiser.
