RN Case Manager - Hospice

Hospice Registered Nurse Case Manager Location: Eaton Rapids County. Employment Type: Full-Time. Sign‑On Bonus: $5,000. Pay and Benefits Collaborative work environment. Job Responsibilities Ensure all activities align with the vision of Elara Caring’s leadership and Hospice team. Assess hospice patients and families to identify physical, psychosocial, and environmental needs using documentation, clinical records, Interdisciplinary Team reports, after‑hours reports, and on‑site evaluations. Obtain data on physical, psychological, social, and spiritual factors influencing patient/family/caregiver health status. Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care. Counsel, instruct, and involve the patient, facility, and family in following the Interdisciplinary Plan of Care. Teach hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospitals, assisted‑living facilities, etc.). Provide appropriate support at time of death and perform bereavement assessment. Qualifications Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate school of nursing. Current State License as a Registered Nurse (RN). At least 1 year of experience in a clinical care setting. Experience in a hospice or home health environment preferred. 50% travel required. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
