The Therapeutic Mentor provides vital support and coaching to children and adolescents with disabilities, in their homes and community - working closely within a multidisciplinary team. This role focuses on behavioral guidance, progress monitoring, and family collaboration to promote skill development and emotional well-being. The mentor utilizes electronic health records and mobile communication tools to maintain clear documentation and effective communication.
Responsibilities
- Support clients in daily activities and therapeutic goals
- Coach behavioral skills to foster positive changes
- Monitor and document client progress using electronic health records
- Collaborate with families and multidisciplinary team members
- Develop client skills to enhance independence
- Communicate effectively with clients, families, and professionals
- Occasionally travel to meet client needs
Preferred Qualifications
- 1+ years experience in therapeutic mentoring
- Associate degree in psychology or related field
- Proficiency in active listening and crisis management
- Knowledge of behavioral therapy techniques
- Strong documentation and communication skills
- Empathy and problem-solving abilities
- Conflict resolution expertise
