Physical Therapist I: Atlantic Health AVN, Full Time, Essex County

JOB DESCRIPTION

Atlantic Health is seeking a dynamic and motivated individual to join the energetic and innovative team of healthcare professionals in their Atlantic Visiting Nurse Department as a Full‑Time Physical Therapist to work 8 AM – 4 PM including one weekend every four to six weeks , the Essex County area. The ideal candidate will evaluate and treat patients in the patient’s home and be a part of a collaborative team of health providers. Principal Accountabilities Performs initial examinations and differential diagnosis. Provides and prescribes plans of care consisting of corrective therapeutic exercise. Performs manual treatments consisting of soft tissue techniques, joint mobilizations, and many more. Supervises and interacts with support staff and maintains proper documentation. Evaluates and establishes a plan of care and treats patients accordingly. Documents patient care and charges appropriately, and documents the course of patient care, including progress made and the ongoing need for treatment. Works with other professionals in an effective manner. Provides clinically appropriate therapeutic interventions. Evaluates patient response to treatment and provides feedback to the interdisciplinary team. Performs other related duties as assigned.

QUALIFICATIONS

Essential Functions/Responsibilities: Evaluate and treat patients in their homes. Minimum Education: High School Diploma or GED, required. Bachelor’s in Physical Therapy, required. Master or Doctorate in Physical Therapy, preferred. Minimum Experience: 1 year of experience in homecare, strongly preferred. License/Certification: NJ License in Physical Therapy, required. BLS, preferred. Valid and unrestricted Driver License with a clean driving record, required.
