Strive Health is seeking a Clinical Lead Nurse Practitioner to manage a defined patient panel across CHF, CKD/ESKD, and chronic conditions. You’ll provide longitudinal, home-based and telehealth care with high-touch clinical expertise and a holistic approach. You will collaborate with interdisciplinary teams, reduce hospitalizations, and advance patient engagement while navigating care transitions and supporting end-of-life planning where appropriate. #J-18808-Ljbffr Strive Health
