Community Health Navigator & Care Coordinator

Variety Care Lafayette

Variety Care Inc. is seeking a Community Health Worker (CHW) to serve as a frontline liaison between patients, care teams, and the community in Oklahoma City. The CHW will improve health literacy, engagement, and access while supporting value-based care and PCMH goals. The role emphasizes relationship-based care, home visits, outreach, and linkage to community resources. A high school diploma or GED and strong communication skills are required; bilingual abilities are a plus. #J-18808-Ljbffr
