Manager - Case Management

Role Overview: Responsible for overseeing case management and discharge planning to ensure high-quality, efficient patient care that complies with regulatory standards. Key Responsibilities: Coordinate daily case management activities, work closely with multidisciplinary teams to optimize patient flow, and ensure compliance with payer guidelines and regulations. Utilize clinical decision-making skills, medical criteria, and discharge planning tools like Interqual or Milliman guidelines. Implement process improvements and monitor performance metrics. Communicate effectively in stressful environments and exercise decisive judgment with minimal supervision. Qualifications & Skills: Minimum of three years of case management experience, with at least two years in a leadership role. Graduate of an accredited nursing program or hold a BSN. Certification in Case Management preferred. Knowledge of hospital information systems, regulatory requirements, and staff development techniques.
