Description

• Conduct evaluations of patients within inpatient or outpatient environments while crafting customized treatment plans
• Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and functionality
• Engage collaboratively with nurses, physicians, case managers, and fellow rehabilitation professionals to optimize patient outcomes
• Provide education to patients and their families regarding therapeutic objectives, discharge plans, and home-based exercises
• Maintain clear documentation of evaluations, treatments, and patient progress per facility and regulatory standards
• Actively participate in interdisciplinary rounds and conferences to ensure seamless coordination of care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications (e.g., neurological, geriatric) are a plus but not obligatory

Skills

• Prior experience is highly preferred
• Able to treat patients with intricate medical and mobility challenges
• Exceptional communication and teamwork capabilities
