As a Stroke Nurse Navigator, you will provide guidance, education, and support to stroke patients and their families. Your role includes coordinating care, enhancing patient experiences, and promoting optimal clinical outcomes. You will work closely with healthcare teams to ensure seamless transitions from hospitalization to rehabilitation, focusing on stroke prevention and patient education. A key aspect of this position is community outreach and engagement in stroke awareness initiatives.

Requirements

• 2 years of experience in acute care or as a nurse navigator
