Description: RN Case Manager Home Health Full Time, Days. You will get to know your patients and become an integral part of their care team. You will receive broad independence while also being part of a cohesive patient-care team. Work/life balance with flexibility is available. You will screen, evaluate, re-evaluate, and treat patients. Coordinate and communicate treatment activities with interdisciplinary team members and patients. Develop a care plan in collaboration with the patient's physician, the patient, and their primary caregivers upon admission. Periodically review and recertify the patient care plan as appropriate. Coordinate the patient's care plan through communication and case conferences as needed. Advise other disciplines of discharges in a timely manner. Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and progress toward goals. Communicate significant changes to the patient's physician and colleagues. Document evaluations, treatment goals, and objectives. Teach patients and caregivers self-care techniques as appropriate. Provide medication, diet, and other instructions as ordered by physicians. Utilize opportunities for health counseling with patients and caregivers.
