Pediatric Transitional Nurse – Registered Nurse (RN) / Licensed Vocational Nurse (LVN) Katy, TX | Bilingual Preferred (Not Required) | Full Time Pay Range: LVN: $30–$35/hour RN: $35–$42/hour What You’ll Do Provide skilled pediatric nursing 1:1 care in patients’ homes Perform patient assessments and ongoing evaluations -Administer medications, treatments, and therapies Support hospital-to-home transitions for high-acuity patients Educate and support parents, families, and caregivers Assist with discharge support and care coordination Train and support permanent field nurses during transitions Provide first-shift and interim coverage as needed Collaborate with Clinical Care Managers and interdisciplinary teams Respond appropriately in emergency situations and changes in patient condition Qualifications Required: Active Texas Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license Current CPR certification Ability to lift and assist pediatric patients as needed At least 3 years of experience with Trach care, Ventilator management, and TPN, post-graduation Home Health Care environment experience High-acuity pediatric patients Emergency response management Schedule & Travel 12 hour shifts (Days or Nights) Flexible scheduling based on patient needs and Nurse availability 5 working days per week, minimum 1 weekend day On call and Staffing coverage responsibilities as needed Local travel within approximately 30–40 miles Angels of Care Pediatric Home Health is a family, clinically-owned company that has been caring for the medically fragile community since 2000!
