Geriatric Dietitian for In-Home Care & PACE

Habitat Health is seeking a dynamic Dietitian to support elderly participants within the PACE model. You will assess nutritional needs, design and implement care plans, and educate participants and caregivers in collaboration with a PCP-led team.
