This role is responsible for evaluation, treatment planning, team collaboration, and supervision of therapy staff to support residents' recovery and mobility goals.Key ResponsibilitiesEvaluate residents within required timeframes and develop individualized treatment plansProvide therapy services in accordance with physician orders and care goalsDocument evaluations, treatments, and progress notes per regulatory standardsSupervise PTAs, aides, and students in compliance with state practice actCollaborate with interdisciplinary team members on resident care plansParticipate in care conferences, utilization review, and rehab meetingsEducate residents, families, and caregivers on treatment plans and discharge needsAssist with durable medical equipment (DME) recommendationsProvide staff training and in-service educationMaintain compliance with all state, federal, and payer regulationsQualificationsDegree in Physical Therapy (Bachelor's required; Master's or Doctorate preferred)Active South Carolina PT license in good standingExperience in skilled nursing or rehab setting preferredFamiliarity with PointClickCare and Casamba preferredStrong clinical, leadership, and documentation skillsWe are an equal opportunity employer.
