Description

• Conduct patient evaluations within both inpatient and outpatient environments and create personalized treatment plans
• Implement physical therapy techniques aimed at enhancing mobility, strength, balance, and overall function
• Work alongside nurses, physicians, case managers, and additional rehabilitation professionals to maximize patient outcomes
• Provide education to patients and their families on therapy goals, discharge planning, and at-home exercises
• Maintain accurate documentation for evaluations, treatment plans, and progress in accordance with facility protocols and regulations
• Engage in interdisciplinary meetings and care discussions to facilitate coordinated patient care

Education

• Doctoral degree in Physical Therapy (DPT) or a comparable qualification from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification is preferred
• Additional certifications (like neurological or geriatric specialties) are advantageous but not mandatory

Skills

• Previous therapeutic experience is highly desirable
• Capable of effectively working with patients having intricate medical and mobility challenges
• Excellent communication and collaboration abilities
• Adept with digital charting and clinical documentation systems
