About The Role We are seeking a compassionate and experienced Social Services Director to join our team at Palm Garden of Mattoon. As a Social Services Director, you will be responsible for leading our social services department and ensuring that our residents receive the highest level of care and support. This is a FULL TIME opportunity that offers the chance to make a meaningful difference in the lives of our residents. Responsibilities Develop and implement social services programs that meet the needs of our residents, including dementia care, hospice, and palliative care Collaborate with other departments, including nursing, therapy, and dietary, to ensure that our residents receive comprehensive care Work closely with direct care staff to support our resident’s social service needs Complete social service assessments timely and attend quarterly care plan meetings Ensure compliance with regulatory requirements and industry standards Requirements 1 year of experience in a social services role, preferably in a skilled nursing home setting is preferred but we train the right candidate High school diploma required.
