About the Role We are seeking a skilled and empathetic Care Companion/Social Worker to join our team. In this part‑time position, you will work closely with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP. Your responsibilities will include: Visiting designated facilities in accordance with the schedule set with your supervisor Conducting assessments of residents to identify emotional, cognitive, and psychosocial needs Implementing short-term, solution-focused therapy and supportive interventions to address immediate concerns Documenting visit notes and maintaining detailed records of interactions and interventions after each visit Collaborating with psychiatric professionals to ensure comprehensive care Qualifications Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills Education: Associate Degree Experience: 1 year Why Join Carease Health?
