Details Home health field setting serving patients across a wide geographic service area in central Texas Caseload of 4–6 patients per day with a target of 30 points per week (equivalent to 40 hrs/week) Patient population includes geriatric and diverse home-based patients requiring skilled nursing care Case types include in-home patient care, interdisciplinary care coordination, and plan of care development EMR: Homecare Homebase (HCHB) and OASIS — experience with both is required; charting must be completed at the patient's home Schedule is Monday–Friday, 8:00 AM–5:00 PM (5x8-hour shifts) Weekend availability required as needed On-call rotation required, including approximately one night of call per week (details discussed at interview) Travel up to 50 miles from home base is required to cover the assigned territory Requirements Active TX RN license or compact RN license valid in Texas — must be in hand at time of consideration (pending licenses not accepted) Current BLS certification required Minimum 1 year of home health experience required; minimum 2 years of overall clinical experience as an RN required HCHB and OASIS experience required; 1 year of experience with both is preferred Ability to travel up to 50 miles from the assigned home base Additional Information Responsibilities include performing comprehensive patient assessments, developing and overseeing plans of care, and coordinating with interdisciplinary teams and certifying providers Non-smokers strongly preferred, as many patients have COPD and are sensitive to cigarette smoke Scrubs in any color are the dress code Local candidates are accepted; candidates must reside within a 50-mile radius of the home base location First-time travelers are welcome to apply Mileage is reimbursed at the IRS standard rate All time-off requests must be disclosed prior to or at the time of interview American Traveler Job ID #P-702852.
