You will provide detailed education to patients and caregivers on home exercise programs and strategies to optimize independence and quality of life.Each day generally includes a series of scheduled home visits, travel between residences, documentation of your findings and interventions in an electronic medical record, and coordination of care with an interdisciplinary team. You will work closely with nurses, occupational therapists, speech therapists, and other clinicians to ensure a cohesive plan of care, timely communication, and strong patient outcomes. Caseloads typically focus on adult and geriatric patients, with visit volumes structured to support thorough, high-quality care and accurate documentation.The environment supports professional growth and the development of clinical expertise in home-based rehabilitation.
