Skilled Nursing Facility (SNF) Nurse

Job Description

We are seeking a dedicated Skilled Nursing Facility (SNF) Nurse with a minimum of 1 year of experience in this specialty. Familiarity with NetHealth (Rehab Optima) is preferred. Candidates must possess an active California nursing license.

Schedule

This position offers a day shift schedule with guaranteed hours of 38 per week.

Responsibilities

• Deliver high-quality nursing care to patients in the SNF setting.
• Develop and implement individualized care plans.
• Administer medications and monitor patient progress.
• Collaborate with a multidisciplinary team to optimize patient outcomes.

Required Experience / Certifications / Licensure

• At least 1 year of experience in a Skilled Nursing Facility (SNF).
• Proficiency in NetHealth (Rehab Optima) is preferred.
• Current California nursing license is required.
