As a Nurse Care Manager, you’ll be at the heart of client wellness, supporting individuals with chronic conditions and behavioral health needs.What You’ll DoCollaborate with an interdisciplinary team to develop client-centered wellness plansConduct health and metabolic screenings (quarterly or annually)Provide outreach calls to engage and support clientsLead health and wellness education for clientsMonitor hospital admissions/discharges and coordinate medication reconciliationPartner with healthcare providers to ensure continuity of careTrack and assess performance measures related to physical healthOccasionally (up to 25%) meet clients in the community or in their homesRequirements of the PositionGraduation from an accredited nursing programActive, unrestricted Missouri LPN or RN licenseValid driver’s license, proof of insurance, and a reliable vehicle(Preferred) 2+ years of nursing experience in a mental health settingFamiliarity with mental illness and substance use disordersStrong communication and customer service skillsComfort with electronic health records and intermediate computer proficiency (Microsoft Office and use of email)Grow With UsWe’re committed to your professional development and will support your growth as a Nurse Care Manager through training, collaboration, and meaningful work.Ready to join a team that’s passionate about people?
