Job Summary

Physical Therapist Home Health (PT HH) – Various Locations. This role involves providing comprehensive home health physical therapy services with an emphasis on geriatric care in a flexible territory within a 60-mile radius of the hub. The position requires a licensed professional capable of delivering patient-centered care in diverse settings, ensuring timely documentation and effective communication.

Job Details

• Certifications, Education, Degree, Experience, and Location Requirements: Valid licensure in-hand; minimum of 1 year of relevant experience; valid driver’s license with personal transportation; BLS certification; recent relevant experience; experience with home health care and HCHB (Homecare Homebase) system required.
• Schedule: Dayshift, 8-hour shifts, minimum of 40 hours per week.
• Responsibilities: Deliver home health physical therapy services focused on geriatric patients, perform charting by 9 am the following day, maintain timely communication, and be flexible with territory.
• Additional Information: On-call rotation may be required; all charting must be completed promptly; travel Pairs accepted with individual territory assignments; modules are billable and completed during orientation; all travelers must have active driver’s licenses and personal transportation.
