Key Responsibilities

• Deliver comprehensive primary care to patients across the lifespan.
• Conduct physical examinations, health assessments, and preventive screenings.
• Diagnose and manage acute illnesses and chronic medical conditions.
• Develop individualized treatment plans based on clinical findings and patient needs.
• Provide patient education on disease prevention, medication management, and healthy lifestyle choices.
• Monitor patient progress and adjust treatment plans to support optimal outcomes.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and promote continuity.
• Follow established clinical guidelines, quality standards, and applicable regulatory requirements.

Required Qualifications

• Active Pennsylvania Family Nurse Practitioner (FNP) license.
• Minimum of two years of experience practicing as a Family Nurse Practitioner.
• Ability and confidence to provide care to patients of all ages.
• Strong clinical assessment, diagnostic reasoning, and treatment planning skills.
• Excellent communication and interpersonal skills.
• Commitment to a collaborative, team-based, patient-centered approach to care.
