Home health allows you to see patients where they are most comfortable-their homes. You'll help individuals recover from illness, injury, surgery, and hospitalization while empowering them to live safely and independently.

What You'll Do

• Evaluate patients and establish individualized treatment plans
• Provide skilled physical therapy services in the home setting
• Assess patient progress and adjust treatment plans as needed
• Educate patients and caregivers on mobility, safety, and recovery goals
• Collaborate with physicians and interdisciplinary team members
• Complete timely and accurate documentation

What We're Looking For

• Active Virginia Physical Therapist License
• Valid Driver's License and Reliable Transportation
• Strong Clinical, Communication, and Documentation Skills
• Home Health Experience Preferred
• Passion for Patient-Centered Care
