This role is ideal for a compassionate, experienced clinician who thrives in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric populations.\n\nWhy You’ll Love Working With Us\n\n\n• Team-Based Care Model: Collaborate with a multidisciplinary team focused on whole-person care—physical, emotional, and social.\n\n• More Time With Patients: Enjoy a lower daily patient volume to foster deeper relationships and deliver more personalized care.\n\n• Supportive Culture: Work in a welcoming, inclusive environment that values teamwork, innovation, and continuous learning.\n\n• Work-Life Balance: Benefit from generous PTO, minimal call responsibilities, and dedicated CME time.\n\n\nKey Responsibilities\n\n\n• Deliver comprehensive outpatient care to adult and senior patients.\n\n• Maintain accurate and timely medical records and documentation.\n\n• Diagnose and manage moderately to complex medical conditions.\n\n• Coordinate referrals and collaborate with specialists as needed.\n\n• Participate in clinical quality improvement initiatives.\n\n• Work closely with interdisciplinary teams to ensure holistic care.\n\n• Contribute to strategic initiatives and innovations in care delivery.\n\n• Exercise independent clinical judgment in patient management.\n\nRequired Qualifications\n\n\n• MD or DO from an accredited medical school.\n\n• Active, unrestricted medical license in the state of practice.\n\n• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.\n\n• Minimum of 2 years’ experience in value-based care or managing high-acuity geriatric patients.\n\n• Commitment to improving patient experience and outcomes.\n\n• Participation in Tuberculosis (TB) screening program.\n\nPreferred Qualifications\n\n\n• Specialty training in Family Medicine, Internal Medicine, Med-Peds, or Geriatrics.\n\n• Experience working with senior populations or in value-based care settings.\n\n• Proficiency with electronic health records (EHR) and digital documentation.\n\n• Strong communication, collaboration, and interpersonal skills.\n\n• Ability to work independently and adapt to evolving clinical environments.\n\n• Experience supervising Advanced Practice Providers (NPs/PAs).\n\nAdditional Information\n\n\n• Full-time, patient-facing role with opportunities for professional growth and leadership.\n\n• Physicians are expected to contribute to a culture of innovation and continuous improvement.\n\n• Competitive compensation package including sign-on bonus, relocation assistance, and comprehensive benefits.\n\nWork Environment\n\n\n• Outpatient clinical setting.\n\n• Standard schedule: Monday–Friday, 8:00 AM – 5:00 PM.\n\nEqual Opportunity Employer\n\n\n\nWe are proud to be an Equal Opportunity Employer.
